Progressive dieback starting at the outer canopy and working in. Smaller leaves each season. Epicormic shoots erupting along the trunk and major limbs — the tree's emergency response to stress. Bark that sounds hollow or sheds in sheets.
None of those are automatically fatal. All of them are worth a look rather than another year of watching.
Easy to confuse, and they need different responses. Frost damage is concentrated on exposed outer growth and on the cold side, and appears after a specific event. Drought stress is more even across the canopy and improves after rain. Fungal decay usually shows at the base or at old wounds.
Getting the diagnosis right matters because two of those three resolve on their own and the third doesn't.
Be sceptical of anyone offering to inject a mature tree back to health. Most real improvement comes from unglamorous things — mulching properly, stopping compaction under the canopy, correcting drainage, and removing deadwood.
Structural decay in the trunk or root plate can't be reversed. At that point the question changes from treatment to how much time the tree has and what's underneath it.
